If I’m not misunderstanding what you’re trying to do, maybe you could add the Disc ID to the existing first medium and then cancel the add-medium edit (which I think will make the second medium disappear immediately).
Thanks for your help! I’m still pretty new to MusicBrainz and honestly, I’m a bit confused by some of the steps you mentioned. I don’t fully understand how to edit the medium or where exactly to find the buttons and pages you’re talking about. I really appreciate your patience with me as I’m learning the system.
If the tracklist of a medium is incorrect, just correct it (Release/Edit/Tracklist) - do not create a new medium.
If the track lengths are incorrect and you have a CD to submit a disc ID (or there’s an already submitted disc ID) - set track lengths from it (Release/Disc IDs) and the lengths will be changed automatically to what is actually on the disc.
You should cancel the edits mentioned above. They are not necessary.
Another thing I noticed:
It looks like track 5 includes a hidden track (see linked Discogs release). This fact should also be reflected in the MB title. It would be either “Those Left Behind / [untitled]” (in case you known this piece has no title) or “Those Left Behind / [unknown]” (if you don’t know the title), or, of course, the title appended, if you know it.

About track 5, I’m trying to edit the title by going to Edit>Recordings>Edit (button next to track) but it wiping out all meta data for that track. Not sure if that’s the right move. Do I have to Add new recording?
You should go to Edit>Tracklist where you can edit the titles. After that, you go to the next tab “Recordings” (where you you can select recordings, belonging to the tracks) and check “Update the recording title to match the track title” to also change the recording title.
On MB we have a tracklist with track titles. Those should be entered as found on your release.
These tracks are linked to recordings (which possibly have many tracks from other releases associated). The titles of recordings should be spelled like printed on the original release. You should not “Update…” if yours is not the original release.
Ok, I did exactly as you said but, track title has not changed.
I did not update, all I wanted to do is correct the track durations. And yes, I have the original copy of the disc. I got it from Cyclone Records themselves. I was one of the band members at the time. Also, Those Left Behind does not have the “Untitled” part at the end, on the back cover. Discogs listed it like that.
A “hidden track” is a track not listed on the release. On CD it’s most often appended to the last track. Then the last MB track title (and recording title) has to be: {listed track} / {hidden track}
Some of your edits are automatically applied, but others will be open for voting. They are applied (and openly visible) after a week, if there are no “No” votes, or a bit sooner if there are at least 3 unanimous “Yes” votes.
Only the last two open edits for Symmetry of Our Plagued Existence should remain. The earlier edits should be cancelled or they will eventually fail because of No votes.

I might be getting off topic but I just listened to a bit and liked what I heard! I’ll probably give the whole album a listen tomorrow. Kind of reminded me of the melancholic atmosphere of early Elend, though guitar-driven rather than symphonic.
