Releases approved with no sources?

A user has entered a “digital media” with no source.

Approaching Normal

This wasn’t a release. It has never been recorded or released.
How do we flag these fake entries on the site to admins?

ETA: I noticed when combing through that release’s edits that the editor took advantage of an exploit that allows releases to be submitted with an empty edit note. That exploit has existed for years, many editors have taken advantage of it and nothing has been done about it.

Raised it on Jira as couldnt find the original.
